Retail Food Inspection Report Conducted January 31, 2019

A Bureau of Food Safety and Laboratory Services inspector from the Pennsylvania Department of Agriculture conducted a retail food inspection at Pho Lavie in Harrisburg on January 31, 2019 and cited the restaurant for 11 violations while, also, determining the eatery was, “Out of Compliance” with requirements set out by the state food safety regulations. 

This is the second consecutive year the restaurant failed their retail food inspection with inspectors finding serious violations in January 2018, as well.

The inspector made the following comments at this most recent inspection:

  • Observed a food employee preparing food without a hair restraint
  • Observed scoops without handles stored in the bulk food bins in the back storage area
  • Food utensils for rice in the back food preparation area observed stored in a container of water which is not maintained at 135°F
  • Observed raw shrimp thawing in standing water, in a plastic basin under a shelving unit in the back food preparation area
  • Observed peeling paint on a wooden table storing food equipment in the grill area
  • The Person in Charge does not have adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection
  • Observed dirty mixing bowls stored directly on the floor under the three compartment sink
  • Observed a plastic container inside the back hand washing sink, indicating use other than hand washing
  • Observed an employee preparing food wearing a bracelet
  • Observed bulk containers of TCS food in the walk-in cooler without any date-marking
  • Observed two smaller fryers in the back food preparation area without a hood system. Grease is accumulating on the walls behind the fryers indicating need for mechanical ventilation
  • Observed personal medicine stored on a shelf above a food preparation table
